- ✪ ✪ ✪ ✪ ✪I run into the same ridiculous issue every single time I'm there. I need a parent concent form for everything, unlesss I'm seeing an OB for some reason, and I have filled out and had this form signed multiple times and it's somehow never there. I'm so sick of having my time wasted. They have to be losing my paperwork.
